事故履帶起重機狀況
此台履帶式起重機由日本石川島播磨重工株式會社於1986年生產,型號為1495—3A,起重量100t,最大起升高度48.5m,起重幅度4.7~39m。設備隨機資料基本齊全,操作人員持有操作證。履帶起重機起重臂墜落後部分損壞。
事故過程及原因分析

從斷裂的驅動鏈銷軸斷口觀察,斷裂麵一側凸出,一側凹進。區域3斷口表麵呈粗粒狀、有光澤、呈灰色,具有過載脆性斷口的特征,該區域為疲勞瞬斷區(失穩擴展區域)。區域2呈細粒狀、灰黑色、無光澤、較平坦,該區域為穩定擴展區域。區域1呈細粒狀、灰黑色、無光澤、且(qie)呈(cheng)台(tai)階(jie)狀(zhuang),有(you)人(ren)字(zi)形(xing)放(fang)射(she)狀(zhuang)花(hua)紋(wen)。這(zhe)是(shi)因(yin)為(wei)在(zai)常(chang)溫(wen)下(xia)由(you)於(yu)過(guo)載(zai)幅(fu)度(du)的(de)突(tu)然(ran)變(bian)化(hua)或(huo)材(cai)料(liao)局(ju)部(bu)機(ji)械(xie)性(xing)質(zhi)的(de)變(bian)化(hua)導(dao)致(zhi)裂(lie)紋(wen)擴(kuo)展(zhan)速(su)率(lv)變(bian)化(hua),而(er)使(shi)斷(duan)口(kou)的(de)痕(hen)跡(ji)或(huo)放(fang)射(she)狀(zhuang)條(tiao)痕(hen)方(fang)向(xiang)改(gai)變(bian),在(zai)宏(hong)觀(guan)斷(duan)口(kou)上(shang)留(liu)下(xia)裂(lie)紋(wen)擴(kuo)展(zhan)的(de)痕(hen)跡(ji),該(gai)區(qu)域(yu)為(wei)啟(qi)裂(lie)區(qu)域(yu),此(ci)處(chu)正(zheng)是(shi)疲(pi)勞(lao)源(yuan)區(qu)所(suo)在(zai)。斷(duan)口(kou)無(wu)明(ming)顯(xian)塑(su)性(xing)變(bian)形(xing),為(wei)典(dian)型(xing)的(de)疲(pi)勞(lao)脆(cui)性(xing)斷(duan)口(kou)。
經驗教訓
(1) 帶式製動器應隨著製動帶的磨損經常予以調整,以防製動力矩不足。
(2) 驅動鏈條作為易損件在日常維護保養中應經常檢查,使用一定時間後應予以更換,以防不測。
(3) 維修不及時,檢查不到位,使安全裝置失效,在關鍵時刻不起作用,引發惡性事故。
(4) 操作人員缺乏經驗,對設備性能了解不夠。
該起重機變幅卷筒工作時,卷筒兩側分別裝有兩套驅動裝置(一側為鏈輪傳動,一側為齒輪傳動)。其中鏈條驅動裝置主要實現起重臂上升(起杆)動作,齒輪傳動裝置主要實現起重臂下降(落杆)動(dong)作(zuo),起(qi)重(zhong)臂(bi)上(shang)升(sheng)與(yu)下(xia)降(jiang)靠(kao)卷(juan)筒(tong)兩(liang)側(ce)的(de)離(li)心(xin)式(shi)離(li)合(he)器(qi)動(dong)作(zuo)進(jin)行(xing)切(qie)換(huan),停(ting)止(zhi)時(shi)靠(kao)帶(dai)式(shi)製(zhi)動(dong)器(qi)保(bao)持(chi)平(ping)衡(heng)。另(ling)外(wai),在(zai)齒(chi)輪(lun)傳(chuan)動(dong)側(ce)還(hai)有(you)一(yi)套(tao)棘(ji)輪(lun)棘(ji)爪(zhao)裝(zhuang)置(zhi),動(dong)作(zuo)後(hou)可(ke)防(fang)止(zhi)變(bian)幅(fu)卷(juan)筒(tong)逆(ni)轉(zhuan)(當製動力矩不足時起重臂在重力作用下下落)。如(ru)果(guo)當(dang)時(shi)操(cao)作(zuo)者(zhe)起(qi)杆(gan)作(zuo)業(ye)結(jie)束(shu)發(fa)現(xian)起(qi)重(zhong)臂(bi)不(bu)能(neng)保(bao)持(chi)停(ting)止(zhi)而(er)下(xia)滑(hua),此(ci)時(shi)不(bu)采(cai)取(qu)起(qi)杆(gan)動(dong)作(zuo)而(er)采(cai)取(qu)落(luo)杆(gan)動(dong)作(zuo),變(bian)幅(fu)卷(juan)筒(tong)落(luo)杆(gan)會(hui)通(tong)過(guo)另(ling)一(yi)套(tao)落(luo)杆(gan)驅(qu)動(dong)裝(zhuang)置(zhi)(齒輪傳動)將動力傳遞到變幅卷筒,可以操作起重臂安全下落。
